Abstract: Immunization is one of the most cost-effective and successful intervention in reducing child and infant morbidity and mortality. Reference to administered vaccines, and timeliness of immunization are important to minimize the number of susceptible for a targeted vaccine-preventable disease at the earliest age possible. The "Child Immunization Database System" is a web-based application for immunization records patterned after the Department of Health's Under Five Clinic Growth Chart. It generates summary reports with respect to immunization of Under Five Clinic patients. In addition, the system is capable of sending SMS messages on a child's follow up reminder to parents, and SMS messages on a child's follow up schedule upon the parents query. The system may be used not only as an application for maintaining immunization records, but as a reminder, and decision support systems as well.
