Abstract: A balanced diet is necessary to ensure that a person has enough energy to perform his tasks and to prevent and alleviate diseases. The Philippine General Hospital Outpatient Dietary Department is responsible for advising their patient on how to eat properly, especially those who have special conditions requiring a different diet. The Outpatient Diet Registry System (ODRS) aims to aid dietitians in providing service. ODRS has three users the Patient, the Dietitian and the System Administrator. The Patient can view a diet plan and the menu assigned to him. He may also update his menu. The Dietitian can update patient records by performing Food Recall, submitting the results of laboratory examinations, and by assigning a diet plan and a menu. He may also update diet plans, food items, representative foods and menus. The Dietitian can also generate important reports such as the Individual Summary Result and the Daily Patient Census. The System Administrator can update accounts for Patients and Dietitians. He may also choose activate or deactivate a Dietitian account.
